Resident Program Fee Agreement
House of Mercy is providing the following resident housing in the sober living facility located at 9314 Zimmerly Ave Duluth, MN for a contracted program fee.
1. House of Mercy requires that all residents agree to be housed for a minimum of 120 days and follow all the rules and guidelines set forth and explained in this agreement. Failure to follow the rules and guidelines can result in immediate termination from the House of Healing.
2. House of mercy does not provide any counseling, nor does the House of mercy restrict the undersigned from making their own personal choices as long as they do not violate the house rules and guidelines.
3. Any personal injury or loss is the sole responsibility of the residents. The House of Mercy shall not be held liable by the undersigned for any injury or loss to the resident or their belongings. By and through their signature the resident acknowledges that they are responsible for any accident or injury and will forever hold harmless the House of Mercy unless the House of Mercy is directly responsible for that loss.
4. House of Mercy strongly encourages all residents to purchase their own rental insurance. House of Mercy insurance policy only covers the dwelling and the business assets located within the dwelling.
5. If you as a resident incur any injury or loss at or outside the House of Mercy, or by another resident, you must report that to staff immediately in writing and full detail of the incident. Failure to do so can result in an immediate termination.
6. House of Mercy is a sober living facility and not rental property therefore you are not a tenant. By your signature you agree that you are not protected by, nor will you invoke any protection of local landlord or tenant laws. If it is found that a local landlord or tenant law might apply to you or your situation you hereby renounce any rights, you may or may not have arising under the same. Simply if you are not working a program of recovery and are not in compliance with house rules you may be directed to leave. All decisions are made on an individual basis with consideration for the entire community and the safety of the house. If your residency is terminated your program deposit will be surrendered and any outstanding balance for damage missing linen or unpaid fees will be billed to you.
7. House of Mercy requires all residents to pay their deposit and program fee upon moving in. The program fee of $700 a month is due on the 1st of each month. A security deposit of $400 will be collected when the resident moves in. All program fees are due by the 5th of each month. A late fee of $30.00 will be assessed on all program fees paid after that time. House of Mercy accepts checks, money orders, cash, or online systems like Cash App or Venmo. Any resident that is consistently late paying their program fee is subject to be removed from the House of Mercy and will not receive a refund of their security deposit.
8. The House of Mercy requires all residents to give a 30-day written notice to end their residency. Any cost associated with the damage to House of Mercy property, the cleaning, or your room, or issued linens will be deducted from your program fee. If you have applied for Section 8, Scattered Site Housing or Low-Income Housing prior to moving into House of Mercy you must inform staff upon moving in.
9. All residents of House of Mercy must agree and understand that they can be tested for drugs or alcohol at any time and for any reason as per their agreement with House of Mercy. Residents have full knowledge that the person or persons administering the tests are not medical personnel. All residents must also be aware that if they test positive or refuse compliance or attempt to cheat or circumvent the tests in any way they may be terminated from House of Mercy and required to leave the premises immediately. Your signature below indicates that you understand and give consent.
